华盛顿县启蒙计划公司致力于为您的孩子及其整个家庭提供优质服务。

华盛顿县 Head Start 项目是经过 MSDE 认证和 OCC 许可的项目。


家庭必须居住在马里兰州华盛顿县，并符合“启蒙计划”收入指南才有资格参加该计划。
接受公共财政援助、SNAP、TCA 或 SSI 的人自动符合参加我们计划的资格。在我们的计划中，儿童无需接受如厕训练。

重要提示：未附上所有收入证明文件，入学申请将不被处理！

2. You may complete the Child Enrollment Application by typing your information into the form, then print the form and drop it off at one of our centers with a copy of all income verification documents.

 IMPORTANT: You must attach a copy of the following items to your Child Enrollment Application as proof of Household Income.   Your application will not be processed without these documents.

  • IRS Tax Form 1040 or W-2 Employment Form

  • Temporary Cash Assistance Award Letter (TCA)

  •  Temporary Assistance to Needy Families Award Letter (TANF)

  •  Supplemental Nutrition Assistance Program (SNAP)

  • Social Security / Disability Award Letter (SSI)

  • Unemployment Benefits Page

  • Foster Care Subsidy Award Letter

  • Proof of any and all Household Income

Please drop off your Child Enrollment Application  at our Enrollment Office located at
Sargent Shriver Head Start Center, 837 Spruce Street, Hagerstown, MD 21740

 

Before your child can attend our program, a Head Start Physical Form, Maryland Health Immunization Form, and  Maryland Lead Blood Test Form must be filled- out by your child's doctor and turned in to our Enrollment Office. Be sure to print forms or pick up the forms when dropping off your Child's Application.
